Sifiliz, cinsel yolla bulaşan hastalıklar arasında sık rastlanan ve bu nedenle de toplum sağlığını etkileyen önemli bir hastalıktır. Latent enfeksiyonlar (yani klinik belirtileri olmayanlar) serolojik testlerle tespit edilir. Önceki yıl içinde edinilen latent sifiliz, erken latent sifiliz olarak anılır; diğer tüm latent sifiliz vakaları, geç latent sifiliz veya süresi bilinmeyen latent sifiliz olarak sınıflandırılır.
Tarama amaçlı olarak en sık flokülasyon temelli Veneral Disease Research Laboratory (VDRL) ve aglütinasyon temelli Rapid Plazma Reagin (RPR) kullanılmaktadır. Spesifik testlerle karşılaştırıldığında yalancı pozitiflik oranları daha yüksektir. Treponemal testler treponemalara özgü antikorları gösterir ve enfeksiyonun ikinci haftasından itibaren ortaya çıkar. En sık kullanılanları TPHA (Treponema Pallidum Hemaglutinasyon Assay) ve FTA-ABS (Floresanlı Treponema Antikor-Absorbsiyon Testi)’dir. TPHA, tedavi olmuş hastalarda ömür boyu pozitif kalmaktadır
Spesifik klinik durumlar alternatif tedavilere izin vermesine rağmen, penisilin tercih edilen farmakolojik tedavi olmaya devam etmektedir. Cinsel partner yönetimi, sifiliznin enfektif aşaması tanısı konmuş bir kişiye maruz kalan kişilerin değerlendirilmesini ve tedavi edilmesini içerir. Tedaviye uygun yanıtı sağlamak için yakın klinik takip ve testlerin tekrarlanması önerilir.
Bu olgu sunumunda kliniğimize başvuran 24 yaşındaki bir kadın hastanın tanı ve tedavi sürecini inceleyerek, sifiliz ile ilgili güncel literatürü derlemeyi amaçladık.
